Liability Waiver and Release Form
1. Acknowledgment of Risks
I understand that participation in art and crafting classes offered by Twisted Tiara Designs Church may involve the use of tools, equipment, materials, and supplies that can
pose certain risks. These may include, but are not limited to:
- Sharp tools (e.g., scissors, craft knives, mosaic cutters)
- Hot tools (e.g., glue guns)
- Heavy or breakable materials (e.g., tiles, glass, ceramics)
- Paints, adhesives, and other crafting substances that may cause allergic reactions or skin irritation
I acknowledge that injuries may occur from slips, falls, improper use of materials, or unforeseen
accidents.
2. Assumption of Responsibility
I voluntarily assume full responsibility for any risks, injuries, or damages that I (or my child) may incur
as a result of participation in these classes. I understand that all reasonable precautions are taken to
ensure a safe environment.
3. Release of Liability
I hereby release, waive, and discharge Twisted Tiara Designs, its instructors, contractors, assistants, volunteers and representatives, from any and all liability, claims, demands, actions, or causes of action arising from any
injury, loss, or damage that I (or my child) may experience during or as a result of participation.
This includes, but is not limited to, injuries caused by negligence, accidents, or the actions of other participants.

4. Medical Authorization
In the event of an injury or medical emergency, I authorize Twisted Tiara Designs or representatives to seek medical assistance for me (or my child). I understand that I am fully responsible for any costs incurred for treatment.

5. Media Release
I grant Twisted Tiara Designs permission to use photos or videos taken during classes that may include me (or my child) for promotional purposes (social media, website, printed materials).

6. Conduct & Safety
I agree to follow all instructions for the use of materials and tools and to behave in a respectful, safe  manner during all class activities. I agree that failure to do so may result in removal from the class without refund.

7. Minor Participation (if applicable)
I certify that I am the parent or legal guardian of the minor named above and that I have the legal authority to sign this waiver on their behalf.

8. Agreement & Signature
By signing below, I acknowledge that I have read, understood, and agree to the terms of this Liability Waiver and Release Form. I understand that this waiver applies for all current and future classes hosted by Twisted Tiara Designs.
